...and I found this little gem, from when I went to Italy. It was at some roadside truck stop, don't know any backstory behind it. Any ideas? :)

i know that back in the day the game demo kiosks that where in department stores where very similar to those of arcade games. one of the majior diffrences is the demo kiosks reset after a set perioud of time where the arcade games just triggered a certain button or function of the emulation.
by the look of this I'de say they just set the console up inside the box and modified the controler. more than likely the buttons on top did nothing. more than likely its just someones effort to make a buck off of people who like to play commercial video games. nice find though. I know that this particular flavor of arcade game mod is hard to come by unless you have upwards of about $2-$3k for the spechialty console arcade enclosers or the technical know how to modify a $400 arcade enclosure to do this.
